Analysis of Derailments 2018-19

S&T

1

S&T staff was working at site and picked up relay from location box which resulted in operation of point under moving wheel.

Intensive counseling of S&T staff has been carried out to perform maintenance work under laid down procedure as per S&T manual.

C&W

1

Culprit wagon loaded i.e. reduced route radius & sharp flange leads to derailed the wagon.

Intensive counseling of C&W staff has been carried out to check the wheel parameters as per the laid down procedure.

Engg

1

Cross level uneven due to poor maintenance of track.

Counseling of Engg staff has been carried out to maintain cross level and other parameter of track as per IRPWM.

Incidental/ miscreant

Due to big stone piece obstruction in the track which hit with cattle Guard resulting Wheel Jumped and derailed.

Gang staff has been counselled to check the track carefully. RPF staff has also been instructed to arrest the miscreants accordingly.

Derailment occurred due to cattle run over and got entangled with culprit wagon.

Fencing has been provided in the section and gangmen staff has been counselled to remove waste grass along with the track.

Derailment occurred due to pandrol clip entangled in nose of crossing

Gang staff has been counselled to check the track carefully.

Rolling down of loco due to not properly secured.

Running staff has been counselled to secure dead loco properly. Ambush checks is being carried out to arrest the week area of ruining staff.

Point was changed by on duty SM without ensuring the clearing the loco.

Intensive counseling of Optg and running staff has been carried out to ensure the proper locking and setting of hand point during performing shunting in yard.

Not Ensured the hand point position and Locking condition of cotter pin.

Comb

Derailment occurred due to careless working of Points Man& LPS during shunting.

F.Rly

Loading party loaded inflammable material and did not obey the terms and conditions as per the contract.

Commercial staff has been counselled to confirm the loading materials as per the loading contract.

Siding

Due to poor maintenance of track in private siding.

Commercial department has been advised to counsel the Pvt sidings staff to highlight the track defect if any in time to avoid such type of cases in future..
